| A permanently sealed copy of any obituary notice which has run in The Advocate newspaper within the past year is available for purchase. To order a memorial, send this completed form with $4.00 for 3" x 8.5" copies or $6.00 for 6.25" x 8.5" copies plus 9% tax per notice (check or money order) to:

If a picture did not run in the obit notice and you would like one included on the laminated copy, you must furnish one picture for each copy ordered. Each must be trimmed to 2" x 2". (We do not make pictures.)
