Here’s the list of required vaccines as published by the Department of Health and Hospitals’ Office of Public Health for children attending kindergarten:
Five doses of DTaP (diphtheria, tetanus, pertussis vaccine) or a minimum of four doses if the fourth dose was received after the fourth birthday;
Four doses of polio vaccine or a minimum of three if the third dose was received after the fourth birthday;
Two doses of MMR (measles, mumps, rubella vaccine) given after the first birthday and separated by at least 30 days;
Three doses of Hep B (hepatitis B vaccine) in which the third dose cannot have been received prior to six months of age;
One does of varicella vaccine (VAR) or a history of having had varicella disease, more commonly called chicken pox.
