Credit Application Form
Here are a few instructions for completing the form.
- Please complete all blanks on the form.
- Be sure to date the application and include your Advocate salesperson's name.
- The account number will be assigned in our office. It will appear on your monthly statement.
- The Credit Manager must approve any changes to this application.
- Personal guaranty is required. Exceptions must be approved by the Credit Manager.
- You will be notified of approval / denial within 3 to 5 days after receipt of application.
